Before I knew what was going on, I fetched up in London (standard). If you fetch up somewhere, you arrive there, especially when you have not planned to go there.

For reasons ~best known to her~, she didn’t change her name (nonstandard).
For reasons best known to herself, she didn’t change her name (standard). If you do not know why someone did something, you can say that they did it for reasons best known to themselves.
